Voice your views on Health and Social Care via West Sussex Links
What is LINK?
LINK (Local Involvement Network) formed in April 2008 and exists through England (our local LINK is LINK West Sussex).
· LINK aims to improve health and social care services by listening to your views on local health and social care services and communicates these to local NHS and Social Service departments.
· LINK is independent from the West Sussex County Council and NHS
· Everyone can get involved
How to get involved:
There are different ways of getting involved and you do not need to commit a vast amount of time.
· The Department of Health have developed a questionnaire to hear your views on which areas of care you would like to receive a greater choice. To have your say please follow the link to the LINK West Sussex website and download the questionnaire. http://www.makesachange.org.uk/cms/site/news/west-sussex/nhs-sussex-needs-your-help.aspx Please note the deadline is 24th October 2011
· There is also a ‘have your say’ questionnaire enabling you to share your story

· Alternatively, you can call 0300 111 0102 to get further information on meetings or to speak to someone.
There are many more questionnaires and surveys online on the LINK West Sussex website: http://www.makesachange.org.uk/area/area-group.aspx?AreaID=9
